If you open a Friendly Health policy via an intermediary, you will not be eligible for the e-gift card.
Your Amazon e-gift card will be sent to you via email once we have confirmed that you have paid your fourth premium. There can be a two week processing period before you receive your e-gift card.
If you cancel your Friendly Health policy or stop making your monthly payments before your fourth premium, you will not receive an e-gift card.
You will receive the value of e-gift card which correlates to the Friendly Health level chosen when you apply.
You will only receive an e-gift card if you provide a valid email address during your application.
Amazon e-gift cards are valid for ten years.
This promotion can be withdrawn at any time.
